You clench your teeth and feel it when you wake up

Bruxism is involuntary clenching or grinding of the teeth, usually during sleep. Many people don’t know they do it until someone hears them or their dentist notices the wear.

  • “People tell me I grind my teeth at night”
  • “I wake up with a stiff jaw”
  • “I have a headache when I get up”
  • “My dentist has warned me about tooth wear”
  • “My masseter muscles look prominent”
  • “I wake up tired even after a full night’s sleep”

You don’t need to decide on treatment online: that’s what the assessment is for.

The difference isn’t the product: it’s where the injection goes and how much is used

Botulinum toxin relaxes the masseter, the muscle that generates the force when you clench. Electromyography records its electrical activity and shows the point where that activity is highest.

The point is located, not estimated

The EMG recording identifies where the masseter is most active before injecting.

The dose is adjusted to your muscle

A very hypertrophied masseter and a less active one don’t need the same dose. That’s why there are two prices, not one.

Performed by a clinical neurophysiologist

Dr Taramundi, Head of Clinical Neurophysiology at Cruces University Hospital, works with electromyography every day.

Botox reduces the force; the assessment looks for the cause

Bruxism is sustained by a cycle that is worth understanding before treating it.

  1. 1

    You clench without realising

At night or during stressful periods, almost always involuntarily.

  2. 2

    The masseter becomes overloaded

Jaw pain, face and neck discomfort and morning headaches appear.

  3. 3

    Tooth wear and poorer sleep

Enamel wears down and you wake up tired even after enough hours of sleep.

“Botox acts on the force of the muscle, not on the cause of bruxism.”

What to know beforehand

That’s why the assessment starts by understanding when you clench and what else is going on: sleep, pain and tooth wear.

It relaxes the muscle; it doesn’t remove the cause

It reduces the force with which you clench. What lies behind it — stress, sleep, your bite — is assessed separately.

It does not replace a night guard

In some cases both are combined. Bring yours or tell us if you use one.

It is not a dental treatment

If the main problem is your bite or a tooth, that is a matter for your dentist.

It is not always indicated

Certain medications and some neurological or muscular conditions rule it out. That’s why we ask first.

If after the assessment the injection is not indicated, we tell you and explain your other options.

Assessment and injection take place in the same appointment. No general anaesthesia or operating theatre is needed, and you walk out afterwards.

Price of Botox for bruxism in Bilbao

The cost depends on the dose your muscle needs, and that is determined by the EMG recording. One vial covers most cases.

  1. 1

    Your case is assessed

When you clench, what you have tried, what medication you take and what your dentist has told you. This is where we decide whether the injection is indicated.

  2. 2

    The masseter is recorded

Electromyography locates the point of greatest muscle activity, which sets where to inject and what dose to use.

  3. 3

    Injection and follow-up

An outpatient procedure, with no general anaesthesia and no time off work. Afterwards the result is reviewed to adjust the dose if the treatment needs repeating.

Frequently asked questions about Botox for bruxism

How much does Botox for bruxism cost?

At MediBilbao Salud, treatment with 1 vial costs €390 and with 2 vials, €520. The price includes the first consultation with a clinical assessment and a 30-minute EMG map, the electromyography-guided injection and the follow-up. The amount needed depends on the strength of your masseters and is determined by the EMG recording, not beforehand.

How long does Botox for bruxism last?

Between 4 and 6 months in most cases. Relief from jaw tension starts to be noticeable after 48 to 72 hours and the maximum effect is reached after two weeks. For severe bruxism, the treatment is usually repeated twice a year.

Does Botox work for bruxism?

Botulinum toxin reduces the contraction of the masseter muscle, which generates the force when you clench. Lowering that force reduces jaw tension, the associated pain and the tooth wear caused by clenching. It does not act on the cause of bruxism: it acts on the force with which you clench. If it isn’t indicated in your case, we tell you at the assessment.

What is EMG guidance and why does it make a difference?

Electromyography records the electrical activity of the muscle and shows the point where that activity is highest. This makes it possible to inject at that precise point and adjust the dose to your muscles, instead of locating it by anatomical landmarks. Here it is performed by a clinical neurophysiologist, for whom EMG is an everyday tool.

What are the side effects?

They are uncommon and mild: discomfort or swelling at the injection site, a temporary feeling of muscle weakness when chewing and, rarely, a slight facial asymmetry that resolves on its own. They are discussed with you before injecting, together with your medication and medical history.

Does it replace a night guard?

Not necessarily. A night guard protects your teeth from wear; Botox reduces the force of the muscle. In some cases both are combined and in others it isn’t needed. If you already use a night guard, tell the team: it changes the assessment.

Will it change the shape of my face?

The aim of the treatment is functional: to reduce the force of the masseter. When the muscle is very hypertrophied, the jawline may look slimmer as it relaxes. The dose is adjusted to the EMG recording precisely to treat what is needed and no more.

Who performs the treatment?

It is performed by Dr Silvia Taramundi Argüeso, clinical neurophysiologist (reg. no. 484808125) and Head of Clinical Neurophysiology at Cruces University Hospital. She works with electromyography every day.

Are there alternatives if it isn’t indicated for me?

Yes, and we explain them. Depending on what we find, you may need a dental assessment, a sleep work-up or jaw-control exercises with speech therapy. The ENT (ear, nose and throat) clinic is in this same centre.

Is there a waiting list?

No. Requests are handled Monday to Friday from 09:30 to 19:30, and the assessment and injection take place in the same appointment, so you don’t need to come twice.

Can I be seen in English?

Appointments are held in Spanish. If you need another language, let us know when you book and we will tell you whether it is possible.
